Getting filler for the first time is exciting — and if we're being honest, a little nerve-wracking. There's a lot of information out there, a lot of before-and-after photos that range from gorgeous to terrifying, and a lot of questions that feel almost too basic to ask out loud.

So let's just answer them.

Here are five things every first-time filler patient should know before their appointment — straight from a derm-trained provider who has had this conversation thousands of times.

  1. Swelling is Normal — and Temporary

This is the one that catches most first-timers off guard.

You will likely leave your appointment looking slightly different than your final result. Swelling is a completely normal part of the filler process — it's your body responding to the injection, not a sign that something went wrong.

What to expect:

  • Mild to moderate swelling in the first 24-72 hours

  • Lips tend to swell the most — sometimes significantly more than the final result

  • Swelling typically resolves within 1-2 weeks

  • Final results are best assessed at the 2-week mark

The golden rule for first-timers: don't judge your results until 2 weeks have passed. What you see in the mirror the day after your appointment is not what you'll see at 2 weeks. Give it time.

Practical tip: Don't schedule your first filler appointment the week before a major event. Give yourself at least 2 weeks of buffer — ideally more.

2. Natural Results Come From Conservative Treatment

Here's something the internet doesn't tell you enough: less is almost always more — especially for first-timers.

The patients who end up unhappy with their filler results are usually patients who had too much done too soon. The patients who love their results for years are almost always the ones who started conservatively, assessed at 2 weeks, and added gradually over time.

What to look for in a filler provider:

  • Medical credentials — PA-C, NP, MD, or DO. Not a certificate from a weekend course

  • Dermatology or plastic surgery background — understanding skin at a clinical level matters

  • Experience with facial anatomy specifically — not just injecting, but understanding what's beneath the surface

  • Before-and-after photos that show natural, believable results across different face shapes and skin tones

  • A provider who will tell you when you don't need something — or when you have too much already

4. There Are Things to Do — and Avoid — Before Your Appointment

Preparing properly for your filler appointment reduces your risk of bruising, swelling, and complications. Here's exactly what to do:

In the week before your appointment, avoid:

  • NSAIDs like Ibuprofen and Advil — these thin the blood and increase bruising risk

  • Fish oil, Ginkgo Biloba, and high-dose Vitamin E — same reason

  • Alcohol — avoid for at least 24 hours before

  • Retinoids and Tretinoin — stop 3 days before

  • Dental appointments — avoid 2 weeks before and after filler treatment

Before your appointment:

  • Arrive with clean, makeup-free skin

  • Consider starting Arnica tablets 1 day before if you're prone to bruising (check with your provider first)

  • Stay well hydrated

  • Eat a meal beforehand — don't come on an empty stomach

What to tell your provider:

  • Any history of cold sores (relevant for lip filler specifically)

  • Any blood thinning medications or supplements

  • Any previous filler treatments and where

  • Any allergies — particularly to lidocaine

5. Fillers Can Be Dissolved — But Prevention is Better

One of the most reassuring things about hyaluronic acid filler is that it's reversible. If you don't love your results, or if filler migrates over time, hyaluronidase — a dissolving enzyme — can break it down safely and effectively.

However — dissolving filler is a process, not a magic eraser. It requires an additional appointment, there can be some swelling during the dissolving process, and it takes time for everything to settle before re-treating.
